NIH and AHRQ just announced that they will issue new “FORMS-E” grant application forms and instructions for grant applications due on or after January 25th, 2018. Please read the following notices for the details:

According to NIH, “The majority of form changes introduced in FORMS-E packages relate to the consolidation of human subjects and inclusion enrollment report information previous collected across multiple forms into a new PHS Human Subjects and Clinical Trials Information form. The new form also expands clinical trial data collection to ensure the appropriate level of information for review and to improve oversight.”

There is going to be a transition period where all parent funding announcements will be expired and reissued with new Funding Opportunity Announcement (FOA) numbers to include the new FOPRMS-E packages and instructions. During the transition, applicants may need to be careful about selecting the appropriate forms package. The reissued FOAs will be posted no later than 60 days before their standard due date. The key transition period for all existing FOAs is going to be between October 25th, 2017 and November 25th, 2017.
