Lab Clearance Information

As of 4/1/2026, the Laboratory Equipment Clearance Form and Laboratory Room Clearance Form are now separate. Please ensure you select the correct form based on whether you are requesting clearance for equipment or for a room. An Environmental Health & Safety (EH&S) clearance is required for all laboratory equipment that will be disposed of, recycled, relocated, or handled by non-laboratory personnel, as well as for research-related rooms/spaces that will be vacated, repaired, or renovated. Before any of these activities occur, laboratory personnel must properly decontaminate the equipment or remove all hazards from the space and submit a completed clearance request to EH&S via the Qualtrics forms linked below or via the sidebar buttons. Low-risk laboratory equipment may be eligible for e-Clearance, while equipment with additional hazard considerations requires an in-person EH&S clearance.
